The doctors hardly ever come out into the patient seating area because a nurse comes out to pick patients up and escort them to the back rooms where the doctors are. The doctors also have a private entrance so they do not walk through the same areas as patients. I think the cleaning person might pick up the pennies or maybe throw them away too .....so I think that puts your ethics question to rest.
